Healthy Aging Grant Services

We are proud to announce new services available through the Healthy Aging Grant Program!

The program aims to address social determinants of health and foster an improved quality of life for older Ohioans, enabling them to remain in their homes and stay connected to their communities, preserve their personal assets, and promote a healthy, independent, active lifestyle.

Healthy Aging Grant Services will be available to residents, upon qualification, in a variety of categories including case management, food assistance, care coordination, transportation assistance, digital assistance, homeowner and renter assistance and more. To be considered for these services, applicants must be at least 60 years of age, a resident of Portage County, and meet the income guidelines of being at or below 300% of the Federal Poverty Level (see chart below). These services will be available through June 30, 2024, or as funds remain available.

Income Guidelines: Citizens at or below 300% Federal Poverty Level that have been impacted in some way by Covid 19.
