Community Health Assessment

The Community Health Assessment is a comprehensive look at the health and well-being of Portage County residents. The purpose of compiling this community health assessment is to identify the needs of the community in order to provide appropriate services.

In January 2015, through the dedication of over 20 local organizations, the Portage County Community Health Needs Assessment was released. Upon review, based on consensus, it was determined that primary data needed to be included in the final report. Through existing partnerships from Portage County Health District, Kent City Health Department, and the Center for Health Affairs primary data was successfully collected to provide a comprehensive Community Health Assessment for Portage County. This was officially re-released on March 10, 2016.

This data-rich assessment provides a comprehensive look at the health and well-being of Portage County. Health-related data includes adults (19 years and older), Youth (ages 12-18), and children (ages 0-11) who participated in a county-wide health assessment survey.

The compiling of the Community Health Assessment for Portage County is the first step necessary for the development of a Community Health Improvement Plan.

This Community Health Assessment would not exist without the financial support of the Portage County Combined General Health District, the Kent City Health Department, and the Center for Health Affairs; or without the assistance of community leaders and many public and private health partners, whose members took the time to carefully plan and carry out the assessment. A special thank you to local school officials who assisted in the assessment process and set aside valuable time that allowed 6th to 12th grade students to participate in this important project.
