Strategic Plan

The Portage County Combined General Health District’s Strategic Plan defines and determines the agency’s roles, priorities, and direction over a span of three years (2016-2019). It is used as a guide for making decisions and allocating resources to pursue strategies and priorities. It also identifies five strategic priorities for PCHD to focus on:

  3. Communication and Education

  4. Financial and Agency Funding

  5. Access to Care
