BMV 5756 Change of Address Form

Atmv-10 The surviving spouse can take title to one or two passenger vehicle (s) not disposed of by Will. This can be done by a Surviving Spouse Affidavit, death certificate and the application portion of the reverse side of the title properly completed.

P-076a Repossession Affidavit (19 KB) This form is used to for voluntary assignment of title by borrower to lien holder.

COC-ATMV-15 - Application(s) for Certificate of Title to a Motor Vehicle (56 KB) This form is used to apply for an Original, Duplicate, Memorandum or Salvage Ohio Certificate of Title to a motor vehicle.

BMV 1173 (Rev. 10/00) - Record Request Form (84 KB) This form is used by individuals or companies requesting information from vehicle record, title, or driver record as required by the Driver's Privacy Protection Act (DPPA) 9/13/97.

BMV 3713 (Rev. 6/95) - Application for Duplicate Vehicle Identification Number (VIN) to a Motor Vehicle (67 KB)  This form is completed by the owner or owner's insurer for the purpose of obtaining a replacement vehicle identification number (VIN) plate or vehicle parts derivative identification number plates or stickers determined by a law enforcement officer upon inspection in accordance with Ohio Administrative Code 4501:1-11-01.

BMV 3717 (Rev. 10/01) - Export Application for a Motor Vehicle (66 KB)  This form is completed when an Ohio titled motor vehicle is to be exported from Ohio to another state or country.

BMV 5008 (Rev. 6/01) - Notarized Written Consent Release of Personal Information (26 KB)  This form is completed by individuals for the purpose of authorizing the Ohio Bureau of Motor Vehicles to release personal information to another party. 

Affidavit for Designation of Beneficiary or Beneficiaries by the Sole Owner for a Motor Vehicle, Watercraft, or Outboard Motor Certificate of Title - BMV 3811 (Rev. 7/02) (27 KB) This form is completed by individuals who upon death, wish to transfer ownership by designating a beneficiary or beneficiaries of a titled vehicle, watercraft, or outboard motor.  This includes ATV's, off-road vehicles, manufactured homes, motor homes, recreational vehicles, and trailers and semi-trailers weighing in excess of 4,000 pounds.

Application for Exemption from Payment of Permissive Tax by Non-Resident Member of the Armed Forces - BMV 5741 (Rev. 1/02) (23 KB) This form is completed by an individual in the armed forces who is not a legal resident of Ohio, but is currently stationed in Ohio.

Power of Attorney (for Certificate of Title) - BMV 3771 (Rev. 6/02) (26 KB) This form is used by an owner of a motor vehicle to allow someone other than the owner to execute the assignment of or make application for certificate of title for a motor vehicle.
